Industry clouds built on a CRM or low-code tool handle that well, since the record model comes prebuilt and you extend it. Push heavy HL7 traffic through one, though, and you’ll notice it was built for sales teams first.

Connected-device and IoMT platforms

Device makers come at this from the other end. The catch is fit. Products that need an unusual database or custom networking for a hospital VPN can burn the saved weeks fighting the vendor’s limits. Check the limits page before the architecture review.

Compliance controls built in — not compliance itself

Audit logging and encryption arrive preconfigured, which removes a big chunk of HIPAA Security Rule groundwork. Compliance still isn’t something you can buy. Auditors usually ask about your risk analysis and workforce training before they get to the hosting vendor’s encryption. Assurance in England works the same way.

Interoperability from day one

With FHIR on launch day, partners can integrate without a bespoke interface. That’s the theory. In practice, FHIR versions and profiles differ, so a US Core app won’t talk to a system built on England’s national profiles without mapping work. Most EHRs also expose only part of their data over FHIR.

Scalability without capacity planning

Nobody has to guess how many servers flu season needs. Managed runtimes are scalable by default, which suits telehealth and consumer apps with spiky traffic. Two limits, though. FHIR stores often come with request-per-second quotas, and a Monday-morning appointment rush can hit them. Autoscaling scales the invoice too.

Lower upfront cost, predictable operations

You don’t buy hardware or hire an ops team before the first user logs in. That means the BAA gets signed before a single health record lands on its servers, including test data copied from production.

Signing it covers less than people assume.
